Ohio’s PASSPORT Medicaid waiver program enables Medicaid-eligible elderly Ohio residents to receive the necessary long-term services and support to remain in their homes or other community settings instead of being admitted to nursing homes.
PASSPORT comprises two main components. The first is a phone-based pre-admission screening to evaluate Medicaid eligibility and assess the required level of care.
The second part of PASSPORT is home care. After determining eligibility, a case manager collaborates with the recipient to design a package of in-home services from local providers.
